Custom Home built by BOLEN CUSTOM HOME BUILDERS, INC. using plans by BOLEN CUSTOM HOME BUILDERS, INC. blueprint designer. Contractor is fully licensed and insured. The following will be provided in contract for your new construction:

I. A CUSTOM BUILT HOME, of approximately ____ s.f and ____ car garage

  1. Sill plate to be 2 x 6’s
  2. Sill seal full width of plate
  3. F.H.A. Jack post with support beam included per code (2 x 10’s)
  4. 2 x 10 floor joists with ¾" plywood UL fir deck, 16" O.C. glued, screwed and nailed, also ¼" lauan board over all vinyled areas.
  5. Floor trusses and truss ties will be 16" O.C. also stud walls will be 16" O.C.
  6. Joists bridging nailed to all floor joists
  7. 7/16" O.S.B. sheathing will be installed on all exterior walls with Tyvek excluding garage. ALL BASEMENT WALLS WILL HAVE 1" STYROFOAM SHEATHING TO GRADE.
  8. Garage will have 7/16" O.S.B.
  9. Exterior walls will be 2 x 6’s placed 16" O.C.
  10. All lumber will be stud grade #2 and better
  11. Roof trusses will be 4/12 pitch placed 24" O.C.
  12. Roof design will be per plan
  13. Roof decking will be ½" O.S.B. oxboard with ply clips
  14. There will be one layer of 15# black felt paper installed with a self-sealing CertainTeed Horizon 3- Tab shingle over paper__________________________ color

NOTICE OF LIEN RIGHTS

AS REQUIRED BY THE WISCONSIN CONSTRUCTION LIEN LAW, BUILDER HEREBY NOTIFIES OWNER THAT PERSONS OR COMPANIES FURNISHING LABOR OR MATERIALS FOR THE CONSTRUCTION ON THE OWNER’S LAND MAY HAVE LIEN RIGHTS ON OWNER’S LAND AND BUILDINGS IF NOT PAID. THOSE ENTITLED TO LIEN RIGHTS, IN ADDITION TO THE UNDERSIGNED BUILDER, ARE THOSE WHO CONTRACT DIRECTLY WITH THE OWNER OR THOSE WHO GIVE THE OWNER NOTICE WITHIN 60 DAYS AFTER THEY FIRST FURNISH LABOR OR MATERIALS FOR THE CONSTRUCTION, AND SHOULD GIVE A COPY OF EACH NOTICE RECEIVED TO HIS MORTGAGE LENDER, IF ANY, BUILDER AGREES TO COOPERATE WITH THE OWNER AND HIS LENDER, IF ANY, TO SEE THAT ALL POTENTIAL LIEN CLAIMANTS ARE DUELY PAID.

This notice must be given within ten days after the first furnishings of labor or materials for the construction upon your land contracted between you as owner and the undersigned as builder.
